Hiace Hit Truck at KM 616 around 9:30 a.m.

A trip by PSS Sleman supporters to Surabaya to watch a football match ended in tragedy. A Toyota Microbus Hiace with license plate AB 7831 E carrying the supporter convoy crashed into a tronton truck with license plate H 1872 AG at KM 616+200 of Lane A of the Ngawi-Kertosono Toll Road, Bajulan Village, Saradan District, Madiun Regency, on Sunday (10/18/2026) at around 9:30 a.m. Western Indonesian Time.

The convoy departed Sleman at around 6:00 a.m. with six Toyota Hiace units heading to the Gelora Bung Tomo (GBT) Stadium in Surabaya to watch the Persebaya Surabaya versus PSS Sleman match. Dimas, one of the injured victims, said the vehicle that crashed was the third of the six in the convoy. He did not witness the moments of the incident because he was asleep.

"The third vehicle in the convoy of six is the one that crashed," Dimas said when met at the emergency department of RSUD Caruban on Sunday.

According to initial police accounts, the Hiace driven by a man identified by the initial H was traveling from west to east. Upon reaching the crash site, the driver was suspected of losing concentration before hitting the rear right side of the tronton truck traveling in the slow lane. This initial suspicion remains part of the police's preliminary investigation and has not been stated as a conclusion.

4 Dead, Injured Treated at RSUD Caruban

Madiun Police Chief AKBP Ridwan Maliki confirmed four deaths from the incident. One victim died at the scene, while three others died while receiving treatment at Caruban Regional Hospital (RSUD Caruban) in Madiun Regency.

"Yes, the death toll rose to four. One died at the scene, the other three at RSUD Caruban while receiving treatment," Ridwan said when confirmed by Detikcom.

Several other victims were injured — some of them reportedly in critical condition — and are still receiving medical treatment at the emergency department of RSUD Caruban. As of this writing, the full identities of the deceased and the latest conditions of all injured victims had not been fully obtained.

The initial suspicion about the cause points to the driver losing concentration, possibly falling asleep while driving. The Head of the East Java VI Ngawi Traffic Police Unit, AKP Dodit Prasetyo, conveyed the suspicion, but police stressed that the exact cause is still under investigation.

Investigation Ongoing, Persebaya Prepares Tribute

The Madiun Police Chief confirmed that the investigation is still underway to determine exactly how the fatal collision occurred. Police officers are still collecting witness statements and examining the scene.

The incident also drew the attention of the Persebaya Surabaya match organizing committee. Committee chairman Ram Surahman said his team had received information about the crash and was still verifying the accuracy of the data with the police.

As a final tribute to the victims, the committee is preparing a one-minute silence before the match kicks off. Players from both teams are also scheduled to wear black armbands during the game.
